Healing By Secondary Intention

Possible Skin Cancer on my Nose means Biopsy and Efudex/Fluorouracil Treatment [Episode 61]

Secondary intention healing involves allowing the wound to heal spontaneously without suturing the edges together. During the healing time the wound needs to be cleaned regularly and ointment is typically applied several times per day to keep the wound moist. Depending on the size of the wound, healing can take weeks to months to complete, but eventually the open wound is covered with a new layer of skin that has grown in from the edges. Wounds that heal by secondary intention typically have more scar contraction and may have a lighter color or slight depression in height compared to surrounding skin. For most locations of the head and neck, secondary intention is not an ideal method of wound closure for these reasons. In specific circumstances, however, secondary intention healing may be recommended as a simple method of wound closure that does not require sutures or additional incisions.

Treating Basal Cell Carcinoma

My Skin Cancer Removal

Several types of treatment can be used to remove or destroy basal cell skin cancers. The options depend on factors such as the tumor size and location, and a persons age, general health, and preferences. These cancers very rarely spread to other parts of the body, although they can grow into nearby tissues if not treated.

All of the treatments listed here can be effective when used in appropriate situations. The chance of the cancer coming back ranges from less than 5% after Mohs surgery to up to 15% or higher after some of the others, but this depends on the size of the tumor. Small tumors are less likely to recur than larger ones. Even if a tumor does recur, it can often still be treated effectively.

What Happens During Mohs Surgery

The procedure is done in stages, all in one visit, while the patient waits between each stage. After removing a layer of tissue, the surgeon examines it under a microscope in an on-site lab. If any cancer cells remain, the surgeon knows the exact area where they are and removes another layer of tissue from that precise location, while sparing as much healthy tissue as possible. The doctor repeats this process until no cancer cells remain.

Step 1: Examination and prep

Depending on the location of your skin cancer, you may be able to wear your street clothes, or you may need to put on a hospital gown. The Mohs surgeon examines the spot where you had your biopsy and may mark it with a pen for reference. The doctor positions you for best access, which may mean sitting up or lying down. A surgical drape is placed over the area. If your skin cancer is on your face, that may mean you cant see whats happening, but the doctor talks you through it. The surgeon then injects a local anesthesia, which numbs the area completely. You stay awake throughout the procedure.

Step 2: Top layer removal

Using a scalpel, the surgeon removes a thin layer of visible cancerous tissue. Some skin cancers may be the tip of the iceberg, meaning they have roots or extensions that arent visible from the surface. The lab analysis, which comes next, will determine that. Your wound is bandaged temporarily and you can relax while the lab work begins.

The surgery team offered Jen a solution. First, she’d undergo surgery to remove the large basal cell carcinoma. Later, Dr. Desai, an expert in reconstructive facial plastic surgery, would repair Jens nose in two stages. The first step would be a forehead flap procedure, surgically moving a section of living tissue from Jens forehead to cover the opening left after the cancer was removed. Then, once the flap had taken hold, he would rebuild Jen’s nose, using his skill to maintain its form and function.

Dr. Desai recalls his first meeting with Jen. It was emotional, he says. She was only 38, which is very young to have so many BCCs. She was understandably worried about how she could go back to work and how her kids would see her.

Doctors and patient talked things through. Despite facing a complex and painful series of surgeries, Jen says, I felt so much calmer. The surgeons were phenomenal. They were so knowledgeable and at the same time had the best bedside manner. They answered every question of mine and my husbands and took their time.

The Cosmetic Effects Of Mohs Surgery On The Nose

Mohs surgery, while effective, is not always the best option for treating skin cancer on the nose. Because Mohs requires a doctor to surgically remove multiple layers of skin from the cancerous spot, patients will be left with a wound when the treatment is over. Once the wound heals, there will likely be a scar. Depending on the size of the wound, some patients may even need to have a skin graft or undergo reconstructive surgery to heal.

Stages 0 I And Ii Maxillary Sinus Cancer

Dry Spot of Skin Leads to Skin Cancer Diagnosis and Nose Reconstruction | Kaiser Permanente

The first step in treating most stage 0, I, or II maxillary sinus cancers is surgery to remove the cancer. In most cases, a maxillectomy is done. Lymph nodes in the neck aren’t removed.

Radiation might be needed after surgery for people with stage I and II cancers, if the cancer has features that make it more likely to come back, such as positive margins , or cancer growing into the area around the nerves .

Radiation is often recommended after surgery for adenoid cystic cancers, even if the margins are negative and there’s no perineural invasion, because they have a high chance of coming back after treatment.

For some aggressive types of maxillary sinus tumors, such as undifferentiated cancer or esthesioneuroblastoma, chemotherapy might be recommended along with radiation treatment.

In cases where surgery to remove the cancer would be risky because of other medical problems, treatment may be radiation therapy alone or chemo given with the radiation.

How Is Skin Cancer Treated

Treatment depends upon the stage of cancer. Stages of skin cancer range from stage 0 to stage IV. The higher the number, the more cancer has spread.

Sometimes a biopsy alone can remove all the cancer tissue if the cancer is small and limited to your skins surface only. Other common skin cancer treatments, used alone or in combination, include:

Cryotherapy uses liquid nitrogen to freeze skin cancer. The dead cells slough off after treatment. Precancerous skin lesions, called actinic keratosis, and other small, early cancers limited to the skins top layer can be treated with this method.

Excisional surgery

This surgery involves removing the tumor and some surrounding healthy skin to be sure all cancer has been removed.

Mohs surgery

With this procedure, the visible, raised area of the tumor is removed first. Then your surgeon uses a scalpel to remove a thin layer of skin cancer cells. The layer is examined under a microscope immediately after removal. Additional layers of tissue continue to be removed, one layer at a time, until no more cancer cells are seen under the microscope.

Mohs surgery removes only diseased tissue, saving as much surrounding normal tissue as possible. Its most often used to treat basal cell and squamous cell cancers and near sensitive or cosmetically important areas, such as eyelids, ears, lips, forehead, scalp, fingers or genital area.

Some patients are hesitant to have skin cancer on the nose surgically removed. Because its such a visible area, it may not be preferable to be left with scarring after surgery. And in the more extreme cases, reconstructive surgery can have a significant impact on a patients daily life.

If you are searching for a non-surgical treatment alternative to Mohs, consider IG-SRT. This leading-edge technology treats Basal cell and Squamous cell carcinoma with a cure rate that is approaching 100% just like Mohs. IG-SRT uses advanced technology to deliver a safe level of radiation directly below the skin, targeting and destroying the cancer cells while leaving the surrounding healthy tissues intact. There is absolutely no cutting involved, no scarring, and no reconstructive surgery after treatment.

Skin Cancer Radiation Treatment

One of the most effective types of radiation to treat skin cancer is high-dose radiation . Thats what is offered by Horizon Medical Services. We treat patients in Monroe, Dade, Broward and Palm Beach counties.

HDR therapy is a better option than surgery for many because of its effectiveness and its ability to spare healthy tissue surrounding the cancer site.

HDR is also relatively painless. There is little downtime and with it, patients avoid the risks that come with surgery.

We know hearing the word cancer is upsetting for skin cancer patients, but if the skin cancer is on the face, sometimes that magnifies a patients emotions, says Dr. Mark Pomper, board-certified radiation oncologist and medical director of Horizon Medical Services. The best thing we can do is assure our patients that our goal is that they are cancer-free and free of any scars after treatment. Our patients trust us to deliver on that goal. They put their faith in us, and thats a big responsibility. When a patient finishes treatment and can confidently look in the mirror and like what they see, thats a win for all of us.

Theyre marvelous people. They helped me, says Elaine, a patient treated by Horizon Medical Services.

First off, performing Mohs surgery or a biopsy or any type of surgery to your skin cancer is not going to spread it through your body. So dont be concerned about thatSecond, Mohs surgery provides the highest cure rate and preserves the greatest amount of normal or healthy tissue. Both of these are very good considerations when youre thinking about treating a skin cancer on the nose.
